At Pathfields we have given a great deal of thought to making sure our layout works for our particular resident group. We offer a homely and welcoming environment conducive to the physical, mental and social wellbeing of our residents.  Many of our residents live with us permanently, others stay with us for a short period of respite care. However long a resident stays with us we always make sure they are at the centre of their care. On arrival each resident is allocated their own personal key worker who makes sure that the individual’s wishes and needs are voiced and heard, and that with the support of the whole team, each person can live an enjoyable, fulfilling life here at our centre. We actively encourage our residents to take part in activities both inside and outside the centre. Many of our activities take place within the centre, for example, sporting events, gardening, arts and crafts but we also make sure that our residents spend ample time away from the centre to reflect daily life. Examples of outings include pub lunches, trips to the zoo or seaside.

Residential Care

Residential Care

At Pathfields, each resident has their own care plan so that they can live a life that is meaningful to them. This can look very different for each person depending upon their own goals and interests and family involvement. We give our residents the opportunity to grow and develop in a nurturing and forward thinking way, together with everyone who is important to the individual. Our timetable includes a range of therapeutic activities aimed at increasing independence in undertaking everyday tasks such as shopping, gardening and budgeting. Timetables change regularly to accommodate residents' changing needs and preferences.

Respite Care

Respite Care

A short stay at Pathfields is available for those individuals who live at home but who could benefit from a change of scene. It also offers a break for families and carers and is an opportunity to have care needs re-assessed and to join in with the activities on offer.
